Dear Year 6 parents,
We are working with a number of schools to support the transition of our Year 6 cohort to high school. Supporting students and families with this transition to high school can include the sharing of information about individual students. This can occur through face to face meetings, surveys or record sharing. If your child is going to attend a Private or Catholic high school in 2023, in order for staff to share information with these schools, we require a ‘Third Party Consent Form – Sharing Student Personal Information’ to be completed. This form is attached to this newsflash. You will need to complete this and return it to your Year 6 teacher, before they are able to respond to requests from Private and Catholic high schools.
Please note, that if your child is attending a State High School, this information sharing consent form is not required as these high schools will be able to access student information through our department platform One School.
